2.2招标内容：多普勒诊断仪一台，尿液分析仪一台。

4.1 请符合资质要求的投标单位的法定代表人于2015年10月20日～2015年10月26日（法定节假日除外）法定工作时间，每天上午9:00-11:00，下午2:30-5:00，在漯河市嵩山路北段舒漫财富中心B栋5楼509室报名。

报名时提供：法定代表人身份证明或法人授权委托书、营业执照副本、医疗器械经营（或生产）企业许可证、税务登记证、组织机构代码、《中华人民共和国医疗器械注册证》；制造商授权证书、产品彩页及“3、投标人资格要求”中需携带的所有证书原件及按顺序装订成册加盖单位公章的复印件贰套（原件复印件应一致）。
