normal align=left>四、响应人应具备的资格条件：

normal align=left>1、具有独立承担民事责任的能力；

normal align=left>2、具有良好的商业信誉和健全的财务会计制度；

normal align=left>3、具有履行合同所必需的设备和专业技术能力；

normal align=left>4、有依法缴纳税收和社会保障资金的良好记录；

normal align=left>5、参加政府采购活动前三年内，在经营活动中没有重大违法记录；

normal align=left>6、具有医疗器械经营（生产）许可证；代理商（经销商）须具有生产厂商对本项目授权书。

normal align=left>五、报名及发售竞争性谈判文件时间：2016年1月20日至2016年1月22日

normal align=left>（上午8：00分至11：30分，下午14：30时至17：30时 ）

normal align=left>六、报名地点：卫辉市公共资源交易管理中心（卫辉市比干大道南段西侧）

normal align=left>七、报名要求：报名时须携带法人身份证明或法人授权委托书、委托代理人身份证原件、营业执照副本、医疗器械生产（经营）许可证 。以上材料需提供有效原件及加盖公章的复印件一份，复印件应注明“与原件一致”。

normal align=left>注：购买了竞争性谈判文件并不视为通过资格审查，资格审查工作在谈判开始后由谈判小组独立负责，未通过资格审查的响应将视为无效响应。
