二、申请人的资格要求：

1.投标供应商应具备《政府采购法》第二十二条规定的条件，提供下列材料：

1）具有独立承担民事责任的能力：具有独立承担民事责任的能力。（提供法人或其他组织的营业执照等证明文件）；

2）具有良好的商业信誉和健全的财务会计制度：具有良好的商业信誉和健全的财务会计制度（提供2021年1月份至今任意一月的财务报表或者提供2020年度财务报表）

3）有依法缴纳税收和社会保障资金的良好记录：有依法缴纳税收和社会保障资金的良好记录（1、提供2021年1月份至今任意一个月社保部门（或税务部门）出具的单位参加社会保险证明的证明文件复印件并加盖公章； 2、2021年1月份至今任意一个月单位依法缴纳税收的证明文件复印件加盖公章。）；

4）履行合同所必须的设备和专业技术能力：投标人必须具有医疗器械生产许可证，或医疗器械经营许可证，或供应商所在地设区的市级食品药品监督管理部门出具的医疗器械经营备案凭证，并且在有效期内。

5）参加采购活动前3年内，在经营活动中没有重大违法记录：参加政府采购活动前三年内，在经济活动中没有重大违纪记录，并没有被列入“信用中国”网站“记录失信被执行人或重大税收违法案件当事人”记录名单，和处于中国政府采购网“政府采购严重违法失信行为信息记录”名单中被禁止参加政府采购活动的供应商（需要提供在“信用中国”网站（www.creditchina.gov.cn）以及中国政府采购网(www.ccgp.gov.cn)查询结果）；
