3.本项目的特定资格要求：

合同包1(南海经济开发区人民医院核酸检测委托服务采购项目)特定资格要求如下:

（1）投标人具有有效的《医疗机构执业许可证》。
（2）投标人未被列入“信用中国”网站(www.creditchina.gov.cn)失信被执行人、重大税收违法案件当事人名单、政府采购严重违法失信行为记录名单；不处于中国政府采购网(www.ccgp.gov.cn)“政府采购严重违法失信行为信息记录”中的禁止参加政府采购活动期间。（以采购代理机构于投标截止日当天资格审查前在上述网站查询结果为准，如相关记录已失效，投标人需提供相关证明资料；未查询到的视为未被列入上述名单）。
（3）单位负责人为同一人或者存在直接控股、管理关系的不同投标人，不得同时参加本采购项目投标。
（4）本项目不接受联合体投标。

电子投标文件提交与解密要求	1.电子投标文件提交时间：北京时间2022年1 月10日00：00至2022年1月30 日9:30。 2.电子投标文件提交方式：投标人须在投标截止前将加密的电子投标文件通过交易系统“佛山市公共资源交易信息化综合平台”成功上传。 3.投标文件解密（开启）：开标时，投标人在2022年1月30 日9:30开始后30分钟内，登录交易系统对其电子投标文件进行解密（现信息化综合平台政府采购进场交易系统远程解密功能已完成开发，只接受远程解密，不接受投标人到开标地点进行现场解密），投标人必须在规定时间内进行解密，逾期未进行解密的将导致投标失败，由投标人自行承担责任。 4.编制电子投标文件的相关软件和工具，投标人可通过以下路径进行下载：佛山市公共资源交易信息化综合平台-登录页面-驱动及操作手册下载-投标文件制作软件下载，技术咨询电话：4009980000。
信用记录查询及使用	按照《财政部关于在政府采购活动中查询及使用信用记录有关问题的通知》（财库﹝2016﹞125号）的要求，根据评审时“信用中国”网站（www.creditchina.gov.cn）、中国政府采购网(www.ccgp.gov.cn)的信息，对各投标人信用记录进行甄别，对列入失信被执行人、重大税收违法案件当事人名单、政府采购严重违法失信行为记录名单的投标人，应当拒绝其参与政府采购活动。联合体成员存在不良信用记录的，视同联合体存在不良信用记录。同时对信用信息查询记录和证据截图或打印存档。
