3.本项目的特定资格要求：

合同包1(延大附院心脑血管病医院内科楼楼顶防水改造维修工程)特定资格要求如下:

（1）具有独立承担民事责任能力的法人、其他组织或自然人，并出具合法有效的营业执照或事业单位法人证书等国家规定的相关证明。？
（2）财务状况报告：提供会计师事务所出具的完整的2021年度审计报告（成立时间至提交响应文件截止时间不足一年的可提供成立后任意时段的资产负债表），或其开标前六个月内银行出具的资信证明（附基本户证明资料），或信用担保机构出具的投标担保函。（以上三种形式的资料提供任何一种即可）。
（3）税收缴纳证明：提供截止至开标时间前六个月内任意一个月的缴费凭据；（依法免税的供应商应提供相关文件证明）。
（4）社会保障资金缴纳证明：提供截止至开标时间前六个月内任意一个月的社会保障资金缴存单据或社保机构开具的社会保险参保缴费情况证明，依法不需要缴纳社会保障资金的供应商应提供相关证明。
（5）供应商须具备建筑工程施工总承包三级及以上资质或防水防腐保温工程专业承包二级及以上资质；
（6）供应商具备有效的国家建设行政主管部门颁发的安全生产许可证；
（7）拟派项目经理具有建筑工程专业二级及以上注册建造师资格及有效的安全生产考核合格证书，且无其他在建项目（提供无在建工程的承诺函）
（8）供应商及项目经理须在“陕西省建筑市场监管与诚信信息发布平台”可查询且无不良记录；
（9）书面声明：参加本次政府采购活动前三年内在经营活动中没有重大违纪，以及未被列入失信被执行人、重大税收违法失信主体、政府采购严重违法失信行为记录名单的书面声明。本项目拒绝被列入失信被执行人、重大税收违法失信主体、政府采购严重违法失信行为的供应商参与。
（10）提供具有履行合同所必需的设备和专业技术能力的承诺函。
（11）？法定代表人授权书及被授权人身份证。（法定代表人直接磋商只须提交其身份证原件）
（12）提供中小企业声明函。
