1.满足《中华人民共和国政府采购法》第二十二条规定;

合同包1(志丹县人民医院购置双能X线骨密度仪设备采购项目)落实政府采购政策需满足的资格要求如下:

7、《财政部国务院扶贫办关于运用政府采购政策支持脱贫攻坚的通知》（财库〔2019〕27号）；
8、其他需要落实的政府采购政策。

3.本项目的特定资格要求：

合同包1(志丹县人民医院购置双能X线骨密度仪设备采购项目)特定资格要求如下:

1、经年检合格的企业法人营业执照（经营范围须与采购内容相符）；
2、开户许可证或基本存款账户信息证明；
3、法定代表人委托授权书（法定代表人参加谈判不需提供,但需提供法定代表人身份证）及被授权人身份证；
4、医疗器械经营许可证书；
5、银行资信证明。截止开标前3个月内；
6、企业完税证明：截止开标前3个月内；
7、根据财政部《关于在政府采购活动中查询及使用信用记录有关问题的通知》（财库〔2016〕125号）要求，供应商需提供【信用中国（www.creditchina.gov.cn）】“失信被执行人”、“重大税收违法案件当事人名单”，【中国政府采购网（www.ccgp.gov.cn）】“政府采购严重违法失信行为记录名单”对企业信用记录查询截图。供应商必须为陕西省政府采购网在册供应商；
8、单位负责人为同一人或者存在控股、管理关系的不同供应商，不得参加同一合同项下的政府采购活动；
9、本项目不接受联合体投标。
