合同包1(空气消毒机等)特定资格要求如下:

(1)代理商参与须具有合法的医疗器械经营许可证或备案凭证（二类）;制造商参与须具有医疗器械生产许可证（进口除外）。 同时提供所投产品的医疗器械注册证。如提供进口产品，须提供投标产品生产厂家（或中国总代理）针对本项目出具的授权书。

(2)提供所投产品的合法来源渠道证明，供应商如提供进口产品，须提供投标产品生产厂家（或中国总代理）针对本项目的授权书。

合同包2(大容量低温离心机)特定资格要求如下:

(1)代理商参与须具有合法的医疗器械经营许可证或备案凭证（二类）;制造商参与须具有医疗器械生产许可证（进口除外）。 同时提供所投产品的医疗器械注册证。如提供进口产品，须提供投标产品生产厂家（或中国总代理）针对本项目出具的授权书。

(2)提供所投产品的合法来源渠道证明，供应商如提供进口产品，须提供投标产品生产厂家（或中国总代理）针对本项目的授权书。

合同包3(全自动酶联免疫分析仪)特定资格要求如下:
