(1)拟参加本项目供应商如为所报设备的制造商，则须提供有效期内的《医疗器械生产许可证》； 如为代理商或经销商，所报设备属于医疗器械第一类管理产品的，则须提供有效期内的《第一类医疗器械备案凭证》；所报设备属于医疗器械第二类管理产品的，则须提供有效期内的《第二类医疗器械经营备案凭证》及设备的《医疗器械注册证》；所报设备属于医疗器械第三类管理的产品，则须提供有效期内的《医疗器械经营许可证》及设备的《医疗器械注册证》。

合同包3(麻醉深度监护仪)特定资格要求如下:
